Students are required to have an updated physical on file prior to beginning any activity throughout the school year.  The physicals are good for 12 months + 30 day grace period.

Forms that need to be filled out by the physician are HERE.  This needs to be brought into the school office once completed so that you may be cleared to participate. 

Guthrie Family Medical Center is offering $25 physicals (or they will run your insurance).

GCH Clinics is also available for sports physicals. They will donate $10 back to the school.
